CREATING A PROFESSIONAL FOUNDATION FOR YOUR PRACTICE
As a Prograna Facilitator, you are responsible for creating a safe, considered and professional space for those you work with.
Before facilitating a workshop, training or session, participants should complete an appropriate Health & Wellbeing Form and provide their consent to participate .
This is more than an administrative process, it forms part of responsible facilitation and allows you to understand whether there is any relevant information that may need to be considered before working with an individual.

WHY DO WE COLLECT HEALTH INFORMATION?
A Health & Wellbeing Form creates a clear record of the information a participant has shared with you at the time of their session or training.

Identify Relevant Health, Wellbeing or accessibility considerations

Make informed decisions about how you facilitate responsibly

Establish appropriate boundaries where necessary

Clear Record Keeping, Safety & Protection for all

Demonstrate appropriate professional standards
Confidentiality & Data Protection
Health Information requires particular care under the UK data - protection law.
As a Prograna facilitator collecting participant information, you are responsible for ensuring the way you collect, access, store and retain that information complies with the applicable data - protection requirements, including the UK GDPR and Data Protection Act 2018.
Relevant
Only collect information that you genuinely need for the purpose of facilitating and maintaing records.
Confidential
Participant information should be treated as private and should not be unnecessarily disclosed to others.
Secure
Digital and/or physical records should be protected against loss, inappropriate access or disclosure.
Purpose Led
Information should only be used for the reason it was originally created.
You should also have an appropriate privacy notice explaining what information you collect, why you collect it, how it is stored and how long it is retained for.
Your Responsibility
Every Prograna Facilitator operates their own independent practice. While Prograna provides resources, guidance and professional standards for facilitating the system, you remain responsible for your own practice.
This includes ensuring you have:
Appropriate professional insurance
Suitable health, consultation and consent forms
Appropriate record keeping
Secure handling of participant information
Appropriate privacy and data-protection procedures
An understanding of any additional requirements set by your insurer or professional body

Your insurance company may also have its own requirements regarding health forms, consent, and how long participant records should be retained. You should always check your individual policy.
